Honorary Exemplary Health Equity Practice Award

2019 Awardee | County of Santa Clara Public Health Department

The County of Santa Clara Public Health Department’s Racial and Health Equity team has facilitated the transformation of policies, practices and procedures across the department. They intentionally moved beyond the data to action, authentic community partnerships, and talking explicitly about race. The Racial and Health Equity and Violence Prevention team became the co-chair and backbone support to the East San José PEACE (Prevention Efforts Advance Community Equity) Partnership. Comprised of 37 partners, PEACE Partnership brings together community residents, Public Health, healthcare organizations, schools, city and county agencies, and others to build a healthy, peaceful, and empowered community by preventing and addressing violence and trauma. PEACE Partnership coordinates organizations from multiple sectors to implement community-driven strategies and is addressing violence through 14 upstream interventions operationalized through six workgroups, such as anti-displacement.
